Work Environment

This position is primarily onsite, conducting home visits in the Port Charlotte area. Key Responsibilities

• Conduct thorough evaluations of patients to determine mobility, function, and physical limitations, followed by the development of tailored treatment plans.
• Implement therapeutic interventions, alongside patient and family education, to promote recovery and independence.
• Maintain and interpret accurate documentation of patient interactions, treatment plans, and progress, complying with all regulatory standards.
• Collaborate with a team of healthcare providers, including physicians and support staff, to ensure a seamless continuum of care.
• Engage in community outreach and marketing efforts to promote services within the local community.
• Supervise and mentor physical therapy assistants and support staff while effectively managing resources.
• Participate in regular staff meetings and continuing education opportunities to enhance professional skills and competencies.

Patient Population / Client Focus

The patients you will care for may range from post-operative orthopedic rehabilitation cases to chronic condition management. This varied population requires you to adapt your approaches continually and develop strong therapeutic relationships with patients and their families. Your role is instrumental in enabling patients to regain their independence and improve their overall physical function.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution is required.
• A valid State License in Physical Therapy is mandatory.
• Strong written and verbal communication skills are essential for effective interaction with patients and healthcare team members.
• Proficiency in word processing, electronic documentation, and other computer skills is expected.

Preferred Experience

• At least two years of clinical experience in an outpatient or home healthcare setting is preferred.

Compensation and Benefits

This position offers a competitive salary range of USD 100,000 – 120,000 per year, commensurate with experience and expertise.
