Vitality Physio-Rehab Center is actively seeking a dedicated and experienced Physical Therapist for a full-time role focusing on home care in Boca Grande, Florida. Work Environment

This role will be based onsite, in the client's home environment within the Boca Grande area, allowing for a more personalized approach to rehabilitation. Our facility operates with a focus on collaboration among team members, ensuring that each client receives comprehensive care. You will engage with physical therapy assistants and other healthcare professionals to foster an effective treatment plan.

Key Responsibilities

Your day-to-day responsibilities will include:

• Directing and implementing individualized physical therapy treatment plans aimed at preventing disability, alleviating pain, enhancing muscle function, and maximizing client performance.
• Overseeing physical therapy assistants to ensure adherence to the outlined treatment protocol and quality of care.
• Conducting thorough evaluations of client functionality through various tests targeting muscle, nerve, and joint abilities, followed by appropriate diagnosis to inform treatment.
• Assessing the home environment to recommend adaptations that foster independence and enhance rehabilitation.
• Providing physical therapy services directly in clients' homes as per physician prescriptions.
• Administering therapeutic exercises and assisting or motivating clients with functional activities tailored to their individual needs.
• Utilizing manual therapy techniques, including traction and massage, alongside the application of physical agents.
• Evaluating and adjusting orthotic and prosthetic devices, recommending modifications to physicians when necessary.
• Contributing to the development and refinement of each client's care plan, ensuring alignment with therapeutic goals and client progress.
• Documenting clinical records meticulously, including initial evaluations, care plans, progress notes, and discharge summaries, ensuring all documentation adheres to regulatory standards.
• Participating in interdisciplinary team meetings to holistically address client treatment strategies.
• Providing education to clients and their families on using therapy devices and the overall therapy program.
• Completing on-site functional assessments aligned with the plan of care and client needs.

Patient Population / Client Focus

The primary patient population will involve individuals ranging from those recovering from surgery to clients with chronic illnesses requiring rehabilitation. The emphasis will be on providing tailored support that fosters independence and improves quality of life. As a Physical Therapist in this home care setting, you will develop profound and impactful relationships with clients and their families, which is integral to successful rehabilitation outcomes.

Qualifications

The ideal candidate will meet the following requirements:

• Graduated from an accredited Doctorate in Physical Therapy (DPT) program.
• Current physical therapist license in good standing to practice in Florida.
• One (1) year of experience working as a physical therapist, particularly in an acute or home care setting.
• Current CPR certification.
• Demonstrated capability for critical thinking and responsible decision-making in complex healthcare situations.
• A strong commitment to clinical excellence, quality care, and integrity.
• Exceptional interpersonal communication skills to effectively engage with clients and their families.
• High energy and motivation to contribute to a collaborative team environment focused on patient outcomes.
• Knowledge of federal, state, and agency regulations related to physical therapy.

Preferred Experience

While a minimum of one year of experience is required, candidates with additional experience in home care and those who demonstrate innovative approaches to physical therapy will be highly regarded. Work with technology platforms for patient management and electronic documentation can provide an advantage in this role.

The compensation package includes a salary range of USD 100,000 – 120,000 per year, which reflects the experience and qualifications of the candidate.
