Work Environment

This role is primarily onsite, taking place in our patients' homes throughout the Roanoke area. Our home care setting provides a unique opportunity to connect with patients one-on-one, tailoring treatment plans to individual needs. Key Responsibilities

• Conduct comprehensive evaluations for patients, which includes reviewing medical histories, performing assessments, and documenting findings accurately.
• Develop and communicate individualized treatment plans to patients and their families, ensuring their involvement in goal setting.
• Implement effective treatment protocols grounded in current best practices and adapt these as needed in response to patient progress and changes in health status.
• Collaborate with other healthcare professionals to ensure coordinated care, particularly in cases requiring multidisciplinary approaches.
• Monitor and document shifts in patients' conditions, modifying care plans as required and ensuring effective communication with medical personnel when necessary.
• Engage in discharge planning and follow-up care to guarantee seamless transitions and continued recovery for patients.
• Utilize effective time management strategies to meet productivity expectations, including maintaining schedules and adhering to required documentation standards.
• Contribute to departmental goals by participating in community outreach, marketing initiatives, and organizational compliance efforts.
• Pursue opportunities for professional development, serve as a clinical mentor to students, and participate in ongoing educational activities.
• Uphold a patient-centered approach by promoting excellent customer experiences and actively seeking to remove barriers to access care.

Patient Population / Client Focus

You will primarily work with a diverse patient population, including individuals recovering from orthopedic surgeries, managing chronic pain conditions, and dealing with neurological deficits. Each patient presents unique challenges, requiring tailored therapeutic interventions to enhance mobility, strength, and functional independence.

Qualifications

• A Bachelor's degree in Physical Therapy or a Doctorate of Physical Therapy (DPT) from an accredited institution.
• Current state licensure as a Doctor of Physical Therapy (DPT) through the Physical Therapy Examining Committee.
• A minimum of two years of clinical experience in a relevant setting.
• Current CPR certification to ensure patient safety and readiness.

Preferred Experience

While a Bachelor's degree is required, preference may be given to candidates with a Doctorate of Physical Therapy (DPT) and additional certifications in specialized therapy techniques or areas such as geriatrics or sports medicine. Experience in home health care settings will also be considered a valuable asset.

Compensation and Benefits

This full-time position offers a competitive annual salary ranging from USD 100,000 to 120,000, commensurate with experience.
