Work Environment

This position is set in a home care environment, where the Physical Therapist will visit patients in their residences throughout the Vinton area. The role demands flexibility and adaptability, as therapists work directly in patients' homes, each presenting their own unique challenges and opportunities. You will collaborate closely with families to help patients navigate their recovery journey while providing supportive and compassionate care.

Key Responsibilities

• Conduct thorough evaluations of patients' physical capabilities and limitations, setting functional goals that guide treatment plans.

• Design and implement customized treatment programs based on physician referrals and patient needs, utilizing evidence-based practices for optimal outcomes.

• Document all patient interactions meticulously, tracking progress through evaluations, treatment reports, discharge summaries, and ensuring compliance with state regulations.

• Supervise and mentor Physical Therapist Assistants (PTAs) and support personnel, enhancing team efficiency and patient care quality.

• Communicate with interdisciplinary professionals, including physicians, nurses, and occupational therapists, to coordinate comprehensive care for patients.

• Provide continual feedback to colleagues and help maintain productivity standards while fostering a positive team atmosphere for professional development.

Patient Population / Client Focus

This role focuses on a diverse patient population, including those recovering from surgical procedures, managing chronic illnesses, or experiencing age-related mobility challenges. The Physical Therapist will cater to individuals ranging from pediatric to geriatric patients, adapting treatment strategies suitable for each age group, and addressing specific health concerns such as stroke rehabilitation, joint replacement recovery, and balance disorders.

Qualifications

• A Bachelor of Science degree in Physical Therapy from an accredited institution.

• Current licensure as a Physical Therapist in Virginia.

• Effective verbal and written communication skills necessary for patient interaction and documentation.

• Proficiency in using digital health records and therapy-related software to manage patient records efficiently.

Preferred Experience

• A minimum of 2 years of experience as a Physical Therapist, ideally within a home health care setting.

Compensation and Benefits

The salary for this position ranges from USD 80,000 to 100,000 annually, based on experience and qualifications.
