Work Environment

This position is onsite and involves working directly within patients' homes. The role is predominantly field-based, allowing for direct engagement with patients in their own environments, which is essential for effective rehabilitation. You will collaborate closely with a multi-disciplinary team, including physicians, nurses, occupational therapists, and speech-language pathologists, ensuring cohesive and comprehensive patient care.

Key Responsibilities

• Conduct thorough evaluations of patients to assess physical function, mobility, and rehabilitation needs, setting specific, measurable goals in alignment with physician recommendations.

• Develop and implement individualized treatment plans, utilizing evidence-based practices that are tailored to each patient's unique situation and progress.

• Document all patient interactions meticulously, including evaluations, treatments provided, progress notes, and discharge summaries, using professional terminology that substantiates the need for skilled therapy.

• Supervise and mentor Physical Therapy Assistants (PTAs) and support staff, ensuring high standards of patient care are maintained and that team members are utilized effectively to achieve treatment goals.

• Collaborate and communicate regularly with the healthcare team about patient status, progress, and any necessary modifications to treatment plans or interventions.

• Engage in continuous education and professional development activities, including training and attending workshops to enhance clinical skills and contribute to team growth.

Patient Population / Client Focus

The physical therapist will work primarily with adult and elderly patients who experience a range of conditions affecting mobility, including post-operative recovery, chronic pain, disabilities, and other health challenges. Qualifications

Compensation and Benefits

This position offers a competitive salary range of $80,000 to $100,000 annually, commensurate with experience.
