Key Responsibilities\n

• Conduct thorough psychosocial assessments of patients and families to evaluate emotional, social, and environmental factors affecting their health and well-being.

• Develop, implement, and monitor individualized treatment plans based on assessed needs, incorporating appropriate therapeutic interventions.

• Collaborate with healthcare professionals, including physicians and nurses, to coordinate patient care effectively and involve patients and families in decision-making processes.

• Facilitate comprehensive discharge planning, ensuring smooth transitions from hospital to home or other suitable settings, which includes interaction with insurers and additional service providers.

• Provide education and counseling to patients and families regarding adaptations to illness and life changes, aiming to promote resilience and coping strategies.

• Leverage crisis intervention techniques, and offer brief or long-term therapy sessions as clinically appropriate.

• Serve as a resource and consultant for other clinical staff, educating them on psychosocial practices and the specific needs of oncology patients.

• Evaluate oncology patients for signs of depression and anxiety, providing timely short-term counseling and support.
\n
• Participate in the on-call rotation, responding to after-hour calls and emergencies requiring immediate social work intervention.

• Facilitate involuntary examinations when necessary, adhering to relevant regulations, and coordinate transfers to Baker Act facilities.

• Contribute to the organization’s visibility by presenting at educational conferences and community outreach opportunities.

• Engage in community services, promoting available resources for cancer survivors and contributing to research initiatives.

You will be primarily serving patients within the oncology spectrum, requiring elevated attention to their psychosocial needs amidst challenging health journeys. Patients range from those experiencing acute health crises to long-term cancer survivors dealing with survivorship issues. Your expertise will guide families through complex emotions connected to their care, equipping them with the resources needed to thrive.

Qualifications\n

• A minimum of a Bachelor's Degree in Social Work from an accredited program.

• Current and valid Licensed Clinical Social Worker (LCSW) certification.

Compensation and Benefits\n

We offer a competitive salary range of USD 70,000 – 100,000 annually, commensurate with experience and qualifications.
