Key Responsibilities\n

• Conduct thorough assessments of clients' mental health to create personalized treatment plans that address their specific needs and goals.

• Design and share a schedule outlining appointments, home visits for in-home observations, and necessary documentation for tracking patient development.

• Organize interdisciplinary team meetings to discuss patient progress, refine treatment strategies, and adapt approaches as necessary.

• Document monthly and quarterly progress notes that provide insight into clients' advancements and areas needing further attention.

• Facilitate training sessions to empower staff and team members with competencies in program implementation and best practices in documentation.
