Key Responsibilities\n

• Conduct ongoing assessments of clients to determine their mental health needs and develop personalized treatment plans, ensuring that each client receives tailored support.

• Facilitate clear communication between all stakeholders in the treatment process, including supervisors, direct care staff, and family members, to monitor clients' progress and discuss adjustments to care.

• Schedule and document appointments, home visits, and sessions effectively to maintain organized records and ensure timely follow-up with clients.

• Coordinate home visits to observe clients in their environment, allowing for comprehensive evaluations and support strategies that fit their unique situations.

• Implement program plans meticulously and follow up on their effectiveness, making necessary adjustments based on ongoing analysis and client feedback.

• Engage actively in interdisciplinary team meetings and relevant discussions, sharing insights and contributing to holistic approaches for client care.

• Complete comprehensive progress notes on a monthly and quarterly basis to track advancements and areas for continued support.
