Key Responsibilities\n

• Act as a vital liaison between clients, their families, and outpatient providers to coordinate care plans that reflect each client's unique circumstances and needs.

• Engage actively with insurance companies for pre-certification and utilization management, facilitating access to necessary services for clients.

• Participate in comprehensive discharge planning and aftercare coordination, ensuring that clients transition smoothly and have the necessary resources post-discharge.

• Deliver therapeutic meal support, emphasizing the development of healthy relationships with food in a compassionate and understanding manner.

• Provide empathetic, affirming care, acknowledging and respecting the varied intersecting identities of each client.

Compensation and Benefits\n
