Key Responsibilities\n

As a Senior LCSW, your day-to-day responsibilities will include:

• Conducting comprehensive psychosocial assessments tailored to hospice patients, aiming to understand their emotional, social, and financial needs.

• Providing both individual and group counseling, focusing on adaptation to chronic illness, grief and loss, and coping mechanisms.

• Collaborating with physicians, nurses, and other healthcare professionals to develop holistic care plans that address the whole patient, including physical, emotional, and spiritual needs.

• Coordinating with local community organizations to facilitate post-discharge services, ensuring a continuum of care tailored to individual circumstances.

• Delivering educational resources and emotional support to families regarding treatment options and community resources available post-care.
\n
• Participating in hospital committees to contribute your expertise on social work-related issues, enhancing service delivery models.

• Engaging in crisis intervention and providing immediate counseling when traumatic situations arise, ensuring emotional support for patients and their families.

• Maintaining accurate and timely documentation to comply with hospital policies and national regulations.

You will primarily serve patients receiving hospice care and support their families during a challenging time in their lives.
