Key Responsibilities\n

• Collaborate with primary care providers to identify and manage patients' mental health and psychosocial issues effectively, contributing to a holistic treatment approach.

• Conduct thorough patient intakes, which include diagnostic assessments and functional evaluations, to recommend tailored treatment plans to the primary care team.

• Deliver brief, focused therapeutic interventions aimed at addressing immediate mental health needs and facilitating ongoing treatment.

• Advise the primary care team on appropriate referrals to specialty mental health services when necessary to enhance patient care continuity.

• Initiate follow-up communications with patients to monitor progress, adjust treatment plans as needed, and develop relapse prevention strategies.

• Identify at-risk patients and collaborate on proactive plans to prevent the exacerbation of their conditions.

• Coordinate the delivery of health services, linking with other treatment providers, and ensuring that patient consent is obtained where applicable.

• Accurately document patients' progress and maintain clinical records, ensuring compliance with agency and federal guidelines.

• Keep primary care providers informed about patient needs and progress, directly contributing to the formulation of effective treatment plans.

• Support patients in making healthier lifestyle choices, providing educational resources and skills for self-management in their treatment journeys.

• Consult with clinic management and fellow team members regarding behavioral services, with a focus on outcome effectiveness and program evaluation.

• Assist patients in complying with any primary care treatments initiated, including strategies to handle medication side effects.

• Uphold the ethical standards outlined in the Code of Ethical Conduct for Clinical Professionals.

• Complete additional duties as assigned to support the operations of the clinic.

Qualifications\n

Compensation and Benefits\n

The position comes with a competitive salary range of USD 87,000 – 105,000 per year, commensurate with experience. Additionally, we provide a sign-on bonus to qualified candidates.
