Key Responsibilities\n

• Actively communicate with physicians and healthcare professionals about patient progress, highlighting needs, concerns, and any significant changes in their mental health status.

• Engage with clients' families and their support systems to develop comprehensive treatment and discharge plans that consider all dimensions of care.

• Maintain accurate and timely documentation, including clinical assessments and progress notes, in accordance with the clinic's policies and procedures.

• Collaborate with a multidisciplinary team to develop and implement individualized treatment plans based on thorough psychosocial assessments and proven therapeutic interventions.

• Facilitate discharge planning and orchestrate care coordination efforts by connecting clients with appropriate regional services and resources to ensure continuity of care.

Patient Population / Client Focus\n

This role primarily involves working with adults facing various mental health challenges, including anxiety, depression, trauma-related disorders, and substance abuse issues. Qualifications\n

• A Master’s degree in Social Work, Counseling, Marriage and Family Therapy, or Psychology.

• Current and active state licensure as an LCSW, LMFT, LPC, or an equivalent designation.

• A minimum of 3 years of experience in an outpatient mental health setting, demonstrating a solid foundation in therapy practices and patient engagement skills.

• Excellent technological proficiency, including familiarity with electronic medical records, word processing software, email communications, and spreadsheet applications.

• Strong organizational skills, with the ability to work independently and manage multiple priorities in a remote environment.

Preferred Experience\n

Experience with telehealth services is highly desirable, as our organization leverages technology to enhance client access to care. Compensation and Benefits\n
