Key Responsibilities\n

• Conduct thorough initial assessments and ongoing evaluations to identify each patient’s unique mental health needs.

• Develop individualized treatment plans in collaboration with other healthcare professionals while ensuring clear communication with patients, their families, and ancillary service providers.

• Schedule and execute home visits as necessary, participating in client observations and case reviews to monitor progress and adjust treatment plans accordingly.

• Keep detailed records of patient interactions, including monthly and quarterly progress notes, ensuring documentation adheres to legal and ethical standards.

• Coordinate with interdisciplinary teams in regular meetings to discuss each patient's treatment approach, progress, and any relevant adjustments.

• Facilitate training sessions to ensure team members maintain competency in program implementation and documentation practices.

• Act as a liaison to the broader mental health community, advocating for resources and improvements that align with patient goals.

• Engage in follow-up consultations and support visits, ensuring patients feel supported and understood throughout their treatment journey.

Patient Population / Client Focus\n

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field from an accredited institution.

• A valid and unrestricted Professional Counselor (LPC) license in Texas.

• A minimum of two years of counseling experience in a clinical setting, where you’ve demonstrated the ability to develop effective therapeutic relationships.
