Key Responsibilities\n

• Organize and execute home visits to conduct in-home assessments, chart reviews, and gather pertinent data on client performance.

• Participate in interdisciplinary team meetings to contribute insights and collaborate on patient care interventions.

• Document monthly and quarterly progress notes to provide clear updates on client advancements and treatment effectiveness.

• Arrange and facilitate training sessions to ensure that all team members are equipped with the requisite skills to implement treatment plans and handle documentation properly.

• Conduct follow-up interviews and observations to continually assess and address the ongoing needs of clients.

• Maintain organized and up-to-date records, ensuring that information is readily available for internal and external stakeholders.

• Act as a liaison between the clinic and the broader mental health community, promoting accessible resources and support.

Qualifications\n
