Key Responsibilities\n

• Conduct thorough evaluations and assessments to understand the unique needs of each patient, guiding them on their mental health journey.

• Maintain effective communication with family members, guardians, and other relevant parties to support patient progress and care coordination.

• Document detailed treatment plans, including appointments, house visits, and observation sessions to monitor patient development comprehensively.

• Organize and perform home visits to complete in-home assessments, review charts, conduct staff interviews, and analyze data as necessary for individualized care.

• Participate in Interdisciplinary Team Meetings to discuss patient progress and collaborate on treatment strategies.

• Prepare and maintain monthly and quarterly progress notes to document changes and advancements in patients' conditions.

• Facilitate training sessions for employees to ensure a high standard of care is maintained and that team members are adept in completing necessary documentation.

• Support the development and implementation of community outreach initiatives aimed at enhancing mental health awareness.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related behavioral health field.

• Active and unrestricted licensure as a Professional Counselor (LPC) in Colorado.

• A minimum of two years of experience in a clinical counseling environment, working directly with clients.
