Work Environment\n

This position is primarily remote, offering flexibility while allowing access to our state-of-the-art electronic medical record (EMR) system. Working from home eliminates commute distractions and enables focus on patient care tasks without the friction of in-office meetings. However, occasional on-site meetings may be required to participate in team collaborations and training sessions aimed at enhancing skills and care approaches.

Key Responsibilities\n

• Conduct comprehensive initial assessments of patients and their families to evaluate mental health needs and psychosocial issues relevant to their care.

• Develop and implement evidenced-based treatment plans tailored to each patient’s goals, preferences, and unique circumstances, ensuring alignment with their overall hospice care plan.

• Engage in individual, couples, family, and group therapy sessions to provide emotional support and guidance, helping patients and loved ones navigate grief, loss, and anticipatory grief related to terminal illness.

• Monitor patient progress over time, adjusting treatment plans as necessary based on evaluations and feedback from ongoing sessions.

• Provide crisis intervention services when necessary and facilitate referrals to emergency services, social services, or other mental health professionals to address pressing needs.

• Maintain meticulous clinical documentation in compliance with state and federal regulations, ensuring that all records reflect current treatment plans and patient progress.

• Collaborate with medical providers, hospice team members, and community organizations to coordinate multifaceted care that encompasses physical, emotional, and social support.
