Key Responsibilities\n

• Conduct thorough age- and culturally-appropriate biopsychosocial assessments to evaluate mental health, substance abuse, and medical needs within a primary care environment.

• Deliver a wide range of behavioral health services, including individual and group counseling, crisis intervention, intake assessments, diagnosis, treatment planning, and referrals for chemical dependency treatment.

• Maintain accurate and timely electronic documentation of mental health evaluations, progress notes, and crisis assessments, ensuring compliance with medical and legal standards.
\n
• Act as a liaison between patients, providers, and community resources, offering social work support and referrals to local agencies including medication assistance programs.
\n
• Collaborate effectively with Quality Improvement teams to develop, implement, and refine processes for data collection and analysis to enhance patient outcomes.

• Assist in escalating patient needs to appropriate medical providers, ensuring timely intervention and continuity of care.

Patient Population / Client Focus\n

Your primary focus will be on patients in hospice care, often facing complex emotional and logistical challenges. This role requires you to provide compassionate and sensitive support to individuals nearing the end of their life, as well as their families. You will guide them through difficult conversations and decisions, assuring that their social, emotional, and psychological needs are met with dignity and respect.

Qualifications\n

• A Master’s degree in Social Work is required along with a valid and relevant state license as a Licensed Clinical Social Worker (LCSW).

• A minimum of three years of post-licensure experience in social work, particularly in mental health or clinical settings.
