As a remote position, you will have the flexibility to work from your home environment, with regular communication and collaboration with a dedicated team of healthcare providers. Utilizing telehealth technology, you will engage with patients and their families, ensuring they receive both psychological support and practical assistance from the comfort of their homes. Key Responsibilities\n

• Deliver thorough psychosocial assessments and follow-up consultations to patients and families receiving hospice care, making sure to apply evidence-based treatment methods.

• Develop individualized care plans that address the emotional, social, and spiritual needs of patients, ensuring their dignity and comfort.

• Facilitate access to community resources, assisting families and patients in navigating medical, emotional, and logistical challenges.
\n
• Function as an integral member of the hospice care team by participating in interdisciplinary meetings, sharing insights, and coordinating care across specialties.

• Monitor and maintain appropriate documentation in compliance with all regulatory requirements while managing patient confidentiality and privacy.

• Provide education and support to family members regarding the bereavement process, facilitating grief support groups as necessary.
\n
• Engage with external resources, such as schools and social service agencies, to help coordinate care and services as needed effectively.

Patient Population / Client Focus\n

This role specifically targets patients in end-of-life care within the Hospice Center, providing a crucial support system for individuals with terminal diagnoses and their families. The successful candidate will also engage with underserved populations and be equipped to address the unique challenges these communities face.

Qualifications\n

• A Master’s degree in Social Work (MSW) with a current licensure as an Independent Clinical Social Worker (LICSW) in Washington State.

• A minimum of three years of relevant clinical experience, particularly in hospice or palliative care settings.

• Comprehensive knowledge of Medicaid, Medicare, and insurance-related matters.
\n
• Strong interpersonal skills and the ability to provide culturally sensitive care to individuals from diverse backgrounds.

• Proven ability to assist individuals in accessing community resources and support systems.
\n
• Certification in CANS is required within two months of hire.
\n

Preferred Experience\n

• Experience working with unhoused populations and understanding their specific challenges.

Compensation and Benefits\n

The salary for this position ranges from $100,000 to $105,000 annually, commensurate with experience and qualifications. In addition to a competitive salary, Unified Care NW offers a comprehensive benefits package that includes:
