Key Responsibilities\n

• Conduct thorough assessments of clients' needs, strengths, and challenges through interviews and consultations, both telephonically and in person.

• Develop tailored and measurable treatment goals for each client, formulating individualized support packages aimed at enhancing their emotional and psychological health.

• Provide crisis intervention when necessary, employing your expertise to navigate urgent situations with professionalism and compassion.

• Monitor clients’ ongoing progress, routinely adjusting treatment plans to ensure their continued effectiveness and addressing any evolving needs.

• Engage in case conferencing to collaborate with multidisciplinary teams, offering insights into treatment strategies and patient care.

• Compile and maintain precise and detailed records of client interactions and treatment outcomes, ensuring compliance with confidentiality and ethical standards.

• Facilitate referrals to external agencies and community resources, guiding clients toward additional support systems for their recovery.

• Attend relevant training sessions and professional development workshops, remaining current on best practices in social work and mental health services.

Qualifications\n
