Key Responsibilities\n

• Evaluate patients' mental health conditions through comprehensive assessments to establish diagnoses and create individualized treatment plans.

• Maintain an active caseload of 25-30 client visits per week, ensuring timely documentation of treatment plans and clinical case notes in compliance with company policy.

• Provide comprehensive therapy services including individual, family, and group therapy sessions, utilizing telehealth technologies to enhance service reach.

• Build rapport with clients by fostering a trusting therapeutic relationship, demonstrating stable, appropriate boundaries while facilitating their journey towards mental wellness.

• Collaborate with case managers, family members, schools, and other healthcare professionals to coordinate holistic care approaches.

• Utilize creative therapy techniques and interventions tailored to individual client goals, adapting methods as needed based on progress and feedback.

As an LMFT at Polara Behavioral Health, your clients will represent a diverse demographic, including children, adolescents, couples, and families facing various mental health challenges. Qualifications\n

• Master's degree in Marriage and Family Therapy or equivalent field from an accredited institution.

• Current and active state licensure as a Licensed Marriage and Family Therapist (LMFT).

• Demonstrated experience completing Diagnostic Assessments (DAs), treatment plans, and clinical case notes per mental health regulations.
\n
• Effective communication skills, both written and verbal, facilitating clear interactions with clients and team members.
\n
• Ability to maintain appropriate professional boundaries and exhibit understanding and empathy towards a diverse client base.
