Key Responsibilities\n

• Conduct thorough bio-psychosocial evaluations to identify barriers to treatment, enhance patient health status, and ensure efficient resource utilization.

• Collaborate with a multidisciplinary team, providing consultations regarding patients’ psychosocial backgrounds to develop comprehensive care plans that promote treatment compliance and patient satisfaction.

• Develop personalized treatment plans and utilize both individual and group therapy modalities to address various psychosocial problems, evaluating the effectiveness of interventions continuously.

• Document all social work activities within patients' medical records in accordance with departmental standards and legal regulations.

• Educate patients and their families on medical and social work recommendations to facilitate understanding and cooperation in the treatment process.

• Assess advocacy needs for each patient and coordinate community resources to enhance social support and self-determination.

• Maintain relevant statistics and reports to track outcomes and fulfill departmental requirements.

• Stay updated on community resources to facilitate patient access as necessary.

• Engage in professional development activities, including attending relevant training to enhance your clinical expertise.

The ideal candidate will work with diverse patient populations, particularly those facing end-of-life challenges and related psychosocial issues. Qualifications\n

• A Master’s Degree in Social Work from an accredited institution.

• A valid and current Licensed Clinical Social Worker (LCSW) license from the State Board of Social Work Licensure.

• Demonstrated knowledge and proficiency in addressing the needs of patients, particularly in health or mental health settings.

Preferred Experience\n

While not mandatory, candidates with six months to one year of clinical experience in hospice care or similar health environments are highly desirable. Compensation and Benefits\n

This position offers a competitive salary range of USD 85,000 to 100,000 annually, commensurate with experience.
