The Intake Coordinator is responsible for, but not limited to:

• Completing initial inquiry forms for all potential home health admissions and forwarding completed forms to the appropriate clinical staff, Director of Clinical Services, and/or Administrator.

• Working to convert qualified referrals into admissions whenever possible.

• Notifying the Administrator when referrals do not convert to admissions.

• Tracking all inquiries and referrals daily and providing a weekly report to the Administrator.

• Notifying the Administrator when referrals do not convert to admissions.

• Tracking all inquiries and referrals daily and providing a weekly report to the Administrator.

• Assisting patients, families, and responsible parties with completing admission paperwork as needed.

• Working collaboratively with appropriate staff to ensure all required written or verbal consents are obtained.

Education:

• Associate degree in a related field preferred.

Professional Experience:

• Five to eight years of related experience preferred.

• Experience in an intake, admissions, healthcare, or related capacity preferred.

• Valid driver’s license and reliable transportation required.

• Must be willing to travel.

• Excellent communication, interpersonal, and organizational skills.
