Key Responsibilities

• Conduct thorough evaluations of patients to determine their functional abilities and limitations, creating individualized treatment plans to address their specific needs.
• Collaborate with healthcare providers to ensure continuity of patient care and provide expert consultation regarding treatment strategies.
• Engage with patients across various demographics, communicating with respect and adhering strictly to our customer service standards.
• Implement and monitor treatment plans, making necessary adjustments based on patient progress and feedback.
• Document patient evaluations, treatment plans, and progress in compliance with applicable laws, rules, and professional standards.
• Participate in community outreach initiatives, quality improvement projects, and program development activities.
• Supervise and mentor support staff, including Physical Therapist Assistants and aides, to promote a cohesive team environment.

Patient Population / Client Focus
