ChristianaCare's Hope Center is seeking a compassionate and autonomous RN to join our Community Impact Health Team. Conduct Annual Wellness Visits, Advance Care Planning visits, Transitional Care Management calls, and chronic disease management interventions. Perform clinical procedures including point-of-care testing, EKGs, medication administration, routine treatments, and patient monitoring. Triage walk-in patients and respond appropriately to urgent patient concerns and crisis situations. Serve as a liaison between patients, providers, specialists, hospitals, and community agencies to ensure seamless communication and continuity of care. Educate patients on wellness, preventive care, medication adherence, and disease self-management. Collaborate with physicians, LPNs, MAs, and interdisciplinary team members to deliver an exceptional patient experience. Support quality, safety, compliance, and accreditation initiatives, including JCAHO readiness, POCT oversight, and Safe Practice Behavior Monitoring. Assist with onboarding, mentoring, competency assessments, and clinical education of Medical Assistants and Licensed Practical Nurses. Participate in quality improvement efforts, chart audits, special projects, and clinical process enhancements. Work Schedule Mon-Fri, day shift with some rotation to late day coverage until 8:00PM. 40 hours per week Weekend and holidays per the guidelines of the department Education and Experience Requirements A graduate of a state approved or National League of Nursing accredited School of Nursing RN license or eligible for licensure to practice in the state Delaware Two years’ experience as a RN, physician office experience preferred BSN preferred; ADN considered with agreement of completion of BSN in 3 years BLS certification is required. Hourly Pay Range: $38.22 - $61.16This pay rate/range represents ChristianaCare’s good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
