Key Responsibilities\n

• Assist patients in accessing vital community resources, including support groups, vocational rehabilitation programs, and housing assistance, to enhance their quality of life.

• Educate patients and their families on available community resources and post-rehabilitation services to facilitate smoother transitions and continued support.

• Take an active role in discharge planning, ensuring appropriate transitions to home or other care settings that promote successful reintegration.
