Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for patients to identify emotional, social, and financial needs, ensuring a holistic approach to care.

• Provide individual and group counseling sessions focusing on adjustment, grief, loss, coping strategies, and other relevant psychosocial issues.

• Facilitate and lead family meetings to discuss treatment options and care plans, providing emotional support and resources for families during transitions.

• Coordinate with community organizations and resources to arrange necessary post-discharge services that ensure continuity of care after hospice.

• Offer education to patients and families regarding emotional wellbeing, treatment options, and available support networks.

• Participate actively in interdisciplinary team meetings, delivering insights and expertise on social work matters to optimize patient care strategies.

• Maintain accurate and timely documentation of patient interactions and care plans in compliance with legal and organizational standards.

• Utilize telehealth platforms to conduct virtual therapy and support sessions, adapting approaches as needed to meet the diverse needs of patients.

Patient Population / Client Focus\n

The LCSW will primarily work with patients who are in hospice care, focusing on individuals with terminal illnesses and their families. Qualifications\n

• A Master’s degree in Social Work from an accredited program is required.
