Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate clients’ mental health concerns, strengths, and resources.

• Facilitate individual, family, and group therapy sessions using evidence-based therapeutic modalities such as Cognitive Behavioral Therapy (CBT), Solution-Focused Therapy, and Trauma-Focused Therapy.

• Develop, implement, and monitor individualized treatment plans that outline therapeutic goals and interventions specific to clients' needs.

• Collaborate with the treatment team to deliver coordinated care that aligns with the best practices in hospice care.

• Provide crisis intervention, risk assessment, and safety planning for clients experiencing acute distress or emergencies.

• Maintain accurate and timely documentation of treatment and interactions in compliance with ethical, legal, and organizational standards.

• Lead group therapy sessions, psychoeducation classes, and support groups, fostering a safe space for sharing and learning.

Patient Population / Client Focus\n

In this role, you will primarily support clients in the hospice setting, focusing on individuals and families coping with end-of-life issues. Qualifications\n

• A Master's degree in Social Work from an accredited program is required.
