Key Responsibilities\n

• Establish rapport with patients and their families swiftly, ensuring clear and effective communication in a non-judgmental, jargon-free manner tailored to their communication preferences.

• Collaborate seamlessly with behavioral health and primary care providers within our diverse professional team to implement integrated approaches to patient care.
\n
• Conduct brief, evidence-based screenings and assessments focused on identifying conditions such as depression, anxiety, and substance abuse, subsequently facilitating referrals for further evaluation and treatment when necessary.
\n
• Manage a caseload of patients, providing follow-up brief interventions that support their healing journey and promoting improved mental health outcomes.
\n
• Utilize the electronic medical record system efficiently, ensuring timely and accurate documentation of patient interactions—delivered within 48 hours of each encounter to enhance integrated health service delivery.
\n
• Create and implement individualized care plans that link patients to an array of vital services while facilitating communication among patients, families, and treatment teams.
\n
• Safeguard patient confidentiality while actively participating in quality improvement efforts, case discussions, and departmental meetings to enhance overall service delivery.
\n
• Provide referrals for patients on prescribed psychiatric medications to our consulting psychiatrist for specialized recommendations.
\n
• Engage proactively with patients and families, returning calls in a timely manner to address appointment requests or concerns.
\n
• Identify community resources and promote access to professional services, educational opportunities, and self-help groups that assist patients in achieving their personal objectives.
\n
• Complete additional responsibilities as required by the evolving needs of our patient populations.

Patient Population / Client Focus\n

This role primarily serves school-aged children and adolescents, many of whom come from underserved or economically challenged backgrounds. You will have the opportunity to work with a diverse population, addressing mental health needs through a trauma-informed care approach. Your expertise will be critical in supporting students who face various challenges, including family crises, academic stressors, and social adjustments.

Qualifications\n

• A Master’s degree in a related Behavioral Health field is a requisite.

• Active LCSW licensure is required for this position.
\n
• CPR or BLS certification is necessary.
\n
• A minimum of three years’ experience in the behavioral health field, preferably in a school environment.

• Cultural competence is essential, particularly in working with the BCHC target populations, including underserved communities and homeless individuals.

• Familiarity with trauma-informed care and a social justice framework is preferred.
