Key Responsibilities\n

• Conduct comprehensive biopsychosocial assessments tailored to the unique needs of each patient, incorporating evaluations for mental health, substance abuse, and medical concerns in the hospice context.

• Deliver behavioral health services to individuals, couples, and families by providing counseling, crisis intervention, treatment planning, and thorough evaluations that lead to actionable outcomes.

• Document mental health evaluations, progress notes, and suicide risk assessments in a timely manner using electronic health record (EHR) systems, ensuring compliance with all regulatory standards.

• Actively participate in interdisciplinary team meetings, advocating for patient needs and collaborating with physicians and care teams to coordinate comprehensive care plans.

• Serve as a resource for social work support and facilitate referrals to community resources, including support groups and medication assistance programs for patients and their families.

• Engage in quality improvement initiatives by collaborating with the Quality Improvement team to implement effective data collection processes tailored for enhanced patient care delivery.

Qualifications\n

• Current, valid state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.

Compensation and Benefits\n
