Key Responsibilities\n

• Establish and maintain trusting relationships with clients, families, and colleagues while continuously demonstrating reliability and professionalism.

• Conduct comprehensive assessments to identify client needs, strengths, and areas for improvement, providing tailored therapeutic interventions accordingly.

• Implement integrated care plans that address the comprehensive needs of clients, ensuring effective communication among clients, families, and other healthcare providers.

• Facilitate access to additional services such as crisis management, family support, and community resources.

• Employ evidence-based therapeutic techniques to support clients through various interventions, focusing on empowerment and positive outcomes.

• Actively participate in team meetings and interdisciplinary collaboration to promote shared goals and enhance service delivery.

• Monitor client progress, adjusting care strategies as needed, to ensure optimal alignment with clinical best practices.

• Create a safe environment that promotes emotional and physical well-being while adhering to safety protocols and reporting any harmful occurrences.

Patient Population / Client Focus\n

This position specifically focuses on clients who are experiencing significant mental health disorders or severe emotional disturbances often associated with terminal illness or chronic conditions within a supportive hospice setting. You will work with individuals and families navigating complex emotions, offering psychotherapy and counseling tailored to their unique circumstances and grief processes.

Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited institution.

• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Texas.

• Expertise in risk assessment, crisis intervention, and therapeutic approaches for diverse populations.

• Exceptional verbal and written communication skills to effectively engage with clients and document client interactions.

Compensation and Benefits\n
