Key Responsibilities\n

• Conduct thorough psychosocial assessments to evaluate mental health concerns, strengths, and resources of patients and families facing end-of-life challenges.

• Provide individual, family, and group therapy utilizing evidence-based treatment modalities to support emotional well-being and coping strategies.

• Develop and implement tailored treatment plans, incorporating specific goals, interventions, and follow-up strategies in alignment with patient needs.

• Facilitate multidisciplinary collaboration with physicians, nurses, and other healthcare professionals to ensure a comprehensive support system for clients and their families.

• Offer crisis intervention services, risk assessments, and personalized safety plans for patients experiencing acute distress.

• Maintain accurate documentation in compliance with ethical and legal standards, contributing to continuous improvement in patient care processes.

• Lead group therapy sessions, psychoeducation classes, and caregiver support groups to foster community and resilience among families.

Patient Population / Client Focus\n

In this role, you will primarily support adults and their families facing life-limiting illnesses. The mental health needs of hospice patients often include coping with grief, anxiety about the future, and navigating the emotional impacts of terminal diagnoses. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Active state licensure as a Licensed Clinical Social Worker (LCSW) is required.

• A minimum of two years of relevant clinical experience in a mental health or outpatient setting, preferably within hospice or palliative care.

Compensation and Benefits\n
