Key Responsibilities\n

• Conduct thorough psychosocial assessments using interviews, observations, and detailed reviews of medical records to unravel the emotional, social, and environmental factors influencing patients’ recovery.

• Deliver counseling and emotional support to patients and their families, addressing challenges such as adjustment issues and overall emotional well-being during the rehabilitation process.

• Collaborate closely with the rehabilitation team to formulate individualized treatment plans that encompass patients' psychosocial needs, ensuring a holistic approach to recovery.

• Assist patients in navigating community resources, connecting them to supportive services such as vocational rehabilitation, housing assistance, and local support groups.

• Provide comprehensive education and support to families regarding available community resources and post-rehabilitation services, helping to ease transitions back into everyday life.

• Actively participate in discharge planning, facilitating smooth transitions for patients returning home or moving to other care settings, maximizing their potential for successful reintegration.

Patient Population / Client Focus\n

The LCSW will support a varied patient population, including individuals recovering from strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. Qualifications\n

Compensation and Benefits\n
