Key Responsibilities\n

• Conduct comprehensive assessments for patients, ensuring personalized care plans are tailored to individual needs and circumstances.

• Provide individual and group therapy sessions, implementing evidence-based practices such as cognitive behavioral therapy (CBT) and mindfulness techniques to support emotional well-being.

• Facilitate crisis interventions and triage services for clients experiencing acute distress, ensuring timely and appropriate support.

• Collaborate closely with the hospice team to coordinate care and communicate effectively regarding patient progress and treatment adjustments.

• Develop outreach and educational activities to promote mental health awareness and wellness initiatives within the community.

• Manage case files and ensure documentation is thorough, timely, and compliant with regulatory standards.

Patient Population / Client Focus\n

You will work primarily with patients receiving hospice care, which requires a deep understanding of grief and loss, end-of-life issues, and supportive practices for both patients and their families. Qualifications\n

• Master's degree in Social Work from an accredited institution is required.

• Current and active LCSW license in Pennsylvania.

Compensation and Benefits\n

The position offers a competitive salary ranging from USD 90,000 to 100,000 annually, commensurate with experience.
