Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify clients' mental health needs, strengths, and available resources, ensuring a holistic understanding of their circumstances.

• Create and implement personalized treatment plans, setting specific goals and interventions, and adjusting plans as necessary based on client feedback and progress.

• Facilitate group therapy sessions, psychoeducation classes, and support groups, focusing on coping strategies and emotional support for clients and their families.

• Work collaboratively with the treatment team to ensure cohesive, client-centered care, addressing any concerns regarding clients’ mental health within the hospice context.

• Provide crisis intervention, conducting risk assessments and creating safety plans in urgent situations to protect client well-being.

• Maintain accurate and timely documentation of client interactions in accordance with ethical guidelines and legal standards, ensuring compliance with healthcare regulations.

Patient Population / Client Focus\n

Our hospice care clientele includes individuals facing terminal illnesses and their families navigating end-of-life decisions. Qualifications\n

• A Master's degree in Social Work from an accredited program.

• Active state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n

The salary for this position ranges from USD 80,000 to 100,000 annually, commensurate with experience and qualifications.
