Key Responsibilities\n

• Engage with physicians and other healthcare professionals to communicate patient progress, concerns, and needs, ensuring a holistic approach to mental health care.

• Maintain accurate and timely clinical documentation, including progress notes and treatment records, adhering to all department policies and ethical guidelines.

• Respond urgently to clients in crisis, providing immediate support and intervention strategies to ensure their safety and well-being both in virtual and, when necessary, in-person settings.

• Conduct regular assessments of clients' progress toward treatment goals and adjust therapeutic strategies as needed to facilitate optimal outcomes.

• Work collaboratively with family members and support systems in the planning and coordination of client treatment and discharge options, emphasizing a family-oriented approach to care.

Patient Population / Client Focus\n

The Licensed Independent Clinical Social Worker will primarily serve clients receiving hospice care, focusing on individuals facing serious illness or end-of-life challenges. Qualifications\n
