Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through patient interviews, clinical observations, and a review of their medical history to ascertain emotional and environmental factors affecting healing.

• Provide individual counseling, emotional support, and problem-solving assistance to patients and their families as they navigate adjustment issues and emotional challenges during rehabilitation.

• Collaborate with a diverse rehabilitation team to formulate and implement personalized treatment plans tailored to each patient's psychosocial needs, aiming for the best possible outcomes.

• Assist patients in utilizing community resources, such as support groups, vocational rehabilitation, or housing assistance programs, to foster supportive environments.

• Educate patients and families about available resources and post-rehabilitation options to facilitate smooth transitions to home or other care settings.

• Participate actively in discharge planning by collaborating with multidisciplinary team members to ensure that patients have a solid plan for their recovery journey.

Compensation and Benefits\n
