Key Responsibilities\n

• Conduct thorough assessments to identify clients’ social, emotional, and environmental challenges, gathering detailed information to inform service plans.

• Develop individualized service plans with clear therapeutic goals and intervention strategies, regularly monitoring client progress and adjusting as necessary.

• Provide evidence-based counseling and emotional support to individuals and families facing challenges such as grief, trauma, or addiction.

• Utilize crisis intervention techniques, conducting risk assessments and safety planning to ensure client well-being and immediate support in critical situations.

• Collaborate with a network of community organizations to coordinate comprehensive care for clients, ensuring they have access to the necessary resources.

• Engage in community outreach efforts to raise awareness about services offered at Legacy Healing Center, aiming to connect with those who may feel isolated or unaware of available support.

• Participate in interdisciplinary meetings and case conferences to share insights and strategies for improving client outcomes.

Patient Population / Client Focus\n

This role involves working primarily with adult and adolescent clients from diverse backgrounds facing a variety of life challenges. Our clients often experience significant barriers, including mental health issues, socioeconomic struggles, and lack of access to supportive services. As a social worker, you will empower these individuals by facilitating access to community resources and providing guidance towards resilience and recovery.

Qualifications\n

• A Master’s degree in Social Work from an accredited program is required.

• Active state licensure as a Licensed Clinical Social Worker (LCSW) in New Jersey.
