Key Responsibilities\n

• Conduct comprehensive assessments of seniors' psychosocial needs, taking into account emotional, mental, and social factors that affect their wellbeing.

• Develop and implement personalized care plans in collaboration with clients and their families, ensuring their preferences and goals are met.

• Provide counseling and emotional support to seniors, guiding them through challenging life transitions such as end-of-life issues or major health changes.

• Advocate for clients’ rights while connecting them with essential community resources to promote their access to services and support.

• Work collaboratively with an interdisciplinary team, including nurses, caregivers, and healthcare professionals, to coordinate comprehensive care strategies.

• Perform regular visits to monitor clients' wellbeing, assess living conditions, and address any emerging concerns related to their care.

• Facilitate effective communication among family members to resolve conflicts and enhance cooperative care for the senior client.

• Educate clients and their families about community resources, support groups, and programs designed to improve their social engagement.

• Maintain detailed and accurate documentation of assessments, care plans, and client progress in compliance with organizational standards.

• Stay updated on current social work practices and regulations to ensure compliance with ethical standards.

Patient Population / Client Focus\n

The primary focus of this role is on the geriatric population, particularly those within our Hospice Center. You will work directly with seniors facing various life challenges, including chronic illness, end-of-life considerations, and social isolation. Your expertise will be pivotal in ensuring these individuals receive the necessary emotional and social support while advocating for their rights and dignity.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
