Work Environment

This full-time position involves working directly in patients' homes, providing personalized treatment in a familiar and comfortable setting. Key Responsibilities

• Conduct comprehensive evaluations to establish baseline function and identify areas of need, crafting tailored treatment plans based on physician referrals.
• Execute effective therapy sessions with patients, utilizing therapeutic techniques and tools designed to enhance mobility and function.
• Document each session meticulously, tracking patient progress and modifying treatment strategies to align with evolving health needs.
• Engage with a multidisciplinary team to discuss patient care strategies and share best practices that can lead to improved recovery outcomes.
• Provide education to patients and caregivers about techniques, exercises, and preventive measures, thereby promoting ongoing health and wellness.
• Meet or exceed utilization goals and other performance metrics to contribute positively to the organization's success.

Patient Population / Client Focus

The Physical Therapist at Motion Care Rehab Center will encounter a diverse patient population, including individuals with orthopedic injuries, pediatric cases, neurologic conditions, and sports-related ailments. Qualifications

• Doctorate degree in Physical Therapy (DPT) from an accredited institution.
• Active state licensure in Physical Therapy.
• Current Basic Life Support (BLS) certification from the American Heart Association.

Compensation and Benefits
