Key Responsibilities\n

• Conduct comprehensive patient intakes by performing diagnostic assessments and functional evaluations, providing informed recommendations for treatment plans to primary care providers.

• Develop and implement brief, targeted interventions to address various mental health needs, ensuring each patient receives the support necessary for their unique situation.

• Regularly monitor patient progress, following up on treatment efficacy and making adjustments to care plans as needed.
\n
• Collaborate with primary care providers to identify patients requiring specialized mental health services and facilitate appropriate referrals to external resources.

• Educate patients on behavioral health strategies, equipping them with self-management skills to foster their recovery and well-being.
\n
• Document all patient interactions and treatment plans meticulously to maintain accurate medical records and ensure continuity of care.
\n
• Assist in the detection of “at-risk” patients, establishing plans to mitigate further risks and enhance stabilization within their daily lives.

• Provide consultation to clinic management and team members regarding behavioral health strategies and program evaluation.
\n
• Adhere to the ethical guidelines set forth for clinical professionals and carry out any additional duties as assigned.

Patient Population / Client Focus\n

Qualifications\n

• A minimum of a Master’s Degree in Clinical Counseling or a related field.

• Valid License as a Licensed Professional Counselor (LPC) in the state of Texas.

• Strong communication skills and the ability to build rapport with clients, their families, and multidisciplinary teams.

• Understanding and application of DSM-IV diagnoses and psychotherapeutic techniques.

Preferred Experience\n

• Bi-lingual proficiency in English and Spanish is highly preferred, facilitating communication with a broader client base.

• Experience in outpatient settings, specifically within Federally Qualified Health Centers (FQHC).

• Demonstrated expertise in group therapy, family therapy, and child psychotherapy.

• Knowledge of community health resources and how to integrate these into patient care plans.

Compensation and Benefits\n
