Som Medical Practice is looking for a passionate and skilled Nurse Practitioner to join our growing team in Niagara Falls, Olean, Lockport, and Hamburg. Responsibilities:

Conduct comprehensive patient assessments, including physical exams, medical histories, and diagnostic evaluations

Diagnose and treat acute and chronic illnesses in alignment with evidence-based clinical guidelines

Develop and manage individualized patient care plans and treatment protocols

Order, interpret, and follow up on laboratory tests, imaging, and other diagnostic studies

Prescribe medications and provide patient education on medication management

Collaborate with physicians, specialists, and care team members to ensure continuity of care

Maintain accurate and timely documentation in the electronic health record (EHR) system

Requirements:

Active Nurse Practitioner (NP) license in the state of New York

Master's or Doctoral degree in Nursing (MSN or DNP) from an accredited program

Current board certification (FNP-C, ANP-C, or relevant specialty certification preferred)

Valid DEA license and prescriptive authority in New York State

Experience in a primary care, internal medicine, or outpatient clinical setting preferred

Proficiency with electronic health record (EHR) systems

Strong interpersonal and communication skills with a patient-centered approach
