Responsibilities
- Plan and conduct segments of treatment programs designed to restore and improve physical function, as well as related social and mental function, in alignment with departmental goals.
- Serve as a co-leader in an interdisciplinary approach to groups of patients, coordinating with team members to support comprehensive care.
- Assess, plan, implement, and modify individual patient treatment plans within the home health setting, documenting progress and adjusting goals as needed.
- Provide patient and family education on prescribed exercises, safety, and home program adherence to support functional outcomes.
- Collaborate with other clinicians (e.g., nurses, aides, and therapists) to coordinate care, ensure continuity, and optimize discharge planning.
- Maintain accurate and timely documentation in patient records, ensuring compliance with regulations and program standards.

Requirements
- Active Physical Therapist license in California
- Ability to practice in a home health care setting
- Strong communication and collaboration skills for interdisciplinary teams
- Proficiency in evaluation, treatment planning, and progress documentation
- Comfort working in patients’ homes and coordinating with families/caregivers
- Willingness to undertake a travel assignment and adhere to schedule requirements
