Key Responsibilities\n

• Conduct in-depth psychosocial assessments through interviews, direct observations, and review of existing medical records to identify the emotional, social, and environmental factors that are affecting students' recovery processes.

• Provide compassionate counseling and emotional support to students and their families, focusing on adjustment issues stemming from health changes, loss, or recovery, fostering resilience and coping strategies.

• Collaborate closely with a multidisciplinary rehabilitation team—including physical and occupational therapists—to co-create individualized treatment plans that incorporate the psychosocial needs of each student.

• Assist students in navigating community resources, such as support groups, vocational training, and housing assistance, thereby promoting sustainable independence.

• Educate students and their families about available community resources and aftercare services necessary for successful transitions out of rehabilitation.

• Participate actively in discharge planning to ensure smooth transitions back to their home environments or into other care settings.

Patient Population / Client Focus\n

As a Licensed Clinical Social Worker at True Living Wellness Center, you will work with a diverse range of student populations, including those recovering from strokes, traumatic brain injuries, orthopedic surgeries, neurological disorders, and psychological conditions. Compensation and Benefits\n
