Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to determine patients' mental health status, identifying their strengths and needs effectively.

• Utilize evidence-based therapeutic modalities such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT) to provide tailored interventions.

• Facilitate collaborative care meetings with multidisciplinary teams to ensure that all aspects of patient wellness are addressed seamlessly.

• Educate patients and their families about mental health diagnoses, outlining treatment options and medication management to promote informed decisions.

• Offer individual, group, and family therapy sessions, adapting approaches to meet age-specific and situational needs.

• Develop individualized treatment plans that reflect the latest therapeutic practices and the preferences of the patient.

• Nurture patient navigation through the mental health system, advocating for their rights and leveraging resources to meet their unique needs.

• Lead therapeutic groups and psychoeducation sessions, as well as participate in discharge planning meetings to ensure continuity of care.

Patient Population / Client Focus\n

In this role, you will work predominantly with individuals and families in end-of-life care settings at our hospice center. This specialized patient population often faces complex emotional and psychosocial challenges related to grief, loss, and transitioning through significant life changes. Your ability to provide empathetic support and clinical expertise will be vital in helping clients and their families navigate these difficult circumstances.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution, ensuring a strong clinical foundation.

• Current active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Arizona, demonstrating compliance with state regulations.

Compensation and Benefits\n
