This is a remote position, allowing you to work from anywhere while connecting with clients across New City, New York. Key Responsibilities\n

• Provide direct clinical services to clients in both office and community settings, focusing on those experiencing major mental illnesses and severe emotional disorders.

• Establish a trusting rapport with clients by demonstrating empathy and cultural sensitivity towards diverse backgrounds and beliefs.

• Engage collaboratively with team members to build and advance common team goals and enhance overall service delivery.

• Conduct comprehensive assessments and screenings, utilizing evidence-based practices tailored to age and developmental needs.

• Create and implement individual care plans, ensuring seamless access to necessary services and effective communication with clients, their families, and other healthcare providers.

• Monitor and manage safety in client interactions, promptly identifying and reporting any potentially unsafe conditions.

• Document all treatments and client interactions comprehensively to meet ethical and professional standards, ensuring timely submissions of data and reports.

• Conduct crisis interventions as necessary and provide ongoing support to clients in distress.

The clients you will be working with primarily include individuals who face significant mental health challenges, particularly those accessing hospice services. You will be tasked with understanding their unique situations, advocating on their behalf, and connecting them to relevant resources that can facilitate their mental health recovery.

Qualifications\n

• A Master's degree in Social Work (MSW) is required, accompanied by an active New York State LCSW licensure.

Preferred Experience\n

• At least 2 years of clinical experience in a mental health setting, preferably working with hospice patients or similar at-risk populations.

• Experience in electronic health record (EHR) documentation and familiarity with data management systems.

Compensation and Benefits\n
