Work Environment\n

This is a full-time remote position with a standard work schedule from Monday to Friday, 8:00 AM to 4:30 PM. While the flexibility to address clients' needs is critical, the job may occasionally require evening or weekend availability, depending on specific client situations. You will conduct assessments and provide support to clients from your home or designated telehealth setting, ensuring seamless communication and care coordination with our interdisciplinary teams, including nurses, caregivers, and other healthcare professionals.
