Key Responsibilities\n

• Serve as a key liaison for families and healthcare providers, facilitating seamless communication and collaboration to enhance patient care.

• Engage with insurance companies regarding pre-certification processes and utilization management to ensure patients receive the necessary care without financial hindrance.

• Participate actively in discharge and aftercare planning, ensuring coherent transitions for patients leaving our hospice center.

• Provide therapeutic meal support, actively engaging clients in modeling healthy relationships with food and fostering their overall well-being.

• Deliver safe, empathetic, and affirming care tailored to each client's unique circumstances, taking into consideration their intersecting identities.

Patient Population / Client Focus\n

Qualifications\n

Compensation and Benefits\n

The salary for this full-time position ranges from USD 90,000 to 105,000 annually, commensurate with experience and qualifications.
