Key Responsibilities\n

• Conduct comprehensive evaluations and assessments of seniors’ psychosocial needs, employing recognized assessment tools and techniques.

• Participate in the development and implementation of Individual Family Service Plans (IFSP), working collaboratively with other healthcare providers.

• Monitor progress and support outcomes regularly through team discussions and client feedback to ensure continuous improvement in care delivery.

• Engage families in counseling and support sessions, equipping them with resources and strategies that promote their well-being and adapt to the challenges of caregiving.

• Maintain compliance with all federal and state regulations, ensuring that our practices meet or exceed required standards.

• Provide crisis management interventions when necessary to assist clients in navigating urgent challenges effectively.
