Work Environment

As a Home Health Physical Therapist, you will operate predominantly on-site, visiting patients in their homes throughout the Tampa area. Our organization values flexibility and adaptability, and your schedule will typically consist of 40 hours per week, with variable weekends as needed to accommodate patient care schedules. You will collaborate with a supportive team of healthcare professionals and have access to a range of resources to ensure the success of your patient interactions.

Key Responsibilities

• Conduct assessments and evaluations of patients' physical abilities and limitations, developing tailored treatment plans to address their unique needs.
• Engage in interdisciplinary collaboration with healthcare providers to ensure continuity of care and optimize patient outcomes.
• Maintain comprehensive documentation of patient treatment throughout the rehabilitation process, adhering to legal and professional standards.
• Implement appropriate therapeutic interventions and re-evaluate treatment effectiveness, making adjustments as necessary to enhance recovery.
• Educate patients and their families on therapeutic practices and home exercises to encourage compliance and independence.
• Participate in community outreach initiatives, program development, and quality improvement efforts to enhance service delivery.
• Supervise support staff and provide mentorship for Physical Therapy Assistants and students.
• Attend and contribute to staff meetings and continuing education programs to foster professional growth.

Patient Population / Client Focus

The patient population you will work with includes individuals experiencing a variety of conditions that affect their movement and functionality, such as orthopedic injuries, neurological disorders, and age-related issues. You will interact with patients of all ages and cultural backgrounds, ensuring a respectful and supportive approach tailored to each individual's circumstances.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• State License to practice Physical Therapy.
• Strong verbal and written communication skills to interact effectively with patients and healthcare team members.
• Proficiency in computer systems and word processing.

Preferred Experience

While a minimum of two years of experience as a Physical Therapist is required, candidates with home health or outpatient therapy experience will be preferred. Familiarity with electronic medical record systems and a background in patient education are also advantageous.

Compensation and Benefits
