Key Responsibilities\n

As a Licensed Professional Counselor (LPC) at Beyond Limits Health System, your primary responsibilities will include:

• Conducting thorough and ongoing assessments to identify and address the mental health needs of your patients.
\n
• Facilitating effective communication amongst supervisors, clinical staff, family members, and other relevant individuals regarding each patient’s progress.
\n
• Developing and sharing a tentative schedule that accounts for appointments, home visits, and observational follow-ups essential for monitoring patient advancement.
\n
• Coordinating in-home visits, which may involve chart reviews, staff interviews, and data collection to ensure comprehensive patient care.
\n
• Overseeing the implementation of treatment plans, ensuring they are executed effectively to meet the goals set for each individual.
\n
• Participating in Interdisciplinary Team Meetings and other vital gatherings to discuss treatment strategies and updates.
\n
• Documenting monthly and quarterly progress notes to track and report on the status and outcomes of each patient’s treatment journey.
\n
• Organizing and leading training sessions for staff to ensure the implementation of treatment plans and documentation practices are executed competently.
\n
• Conducting follow-up interviews, home visits, and direct observations to provide continued support tailored to individual patient needs.
\n
• Maintaining meticulous records that ensure critical information is accessible to authorized personnel and stakeholders.

• Acting as a liaison to the broader community, fostering relationships that support our organization’s development goals.

• Assisting in the creation of procedural training materials designed to enhance program effectiveness for individuals served by the system.

Patient Population / Client Focus\n

This role primarily focuses on working with a diverse array of individuals, including both adults and youth grappling with various mental health challenges. Your expertise in tailoring counseling approaches to meet unique needs will be invaluable in fostering recovery and resilience within our patient population.

Qualifications\n

To be considered for this position, candidates must possess:

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely related field.
\n
• An active and unrestricted license as a Professional Counselor (LPC) in the state of Arizona.
\n
• A minimum of two years of counseling experience, preferably within a clinical environment.
\n
• A strong grounding in family therapy, couples therapy, substance misuse treatment, or trauma-informed care is highly desirable.
\n
• In-depth knowledge of legal and ethical standards relevant to counseling practice.
\n
• Exceptional interpersonal and communication skills, enabling effective rapport-building with a variety of clientele.

• Demonstrated ability to collaborate successfully within a multidisciplinary healthcare team.

• Proficiency in using Electronic Medical Records (EMR) software and related technologies for documenting patient interactions and treatment plans.

Preferred Experience\n

Compensation and Benefits\n
